It looks like it's just one of the chapter title pages, so that's probably why it wasn't in any of the adaptations. It's basically the manga equivalent of a book cover; it's not really canon or relevant to the plot.

I think it's from the light novel "Dance a merry Christmas", but I haven't been able to download a copy with the chapter illustrations to confirm.

Also I seem to remember seeing another FMP illustration with Kanami's best friend chained to something with a booby-trap bomb. I could of sworn I've got a copy on my computer, but for the life of me I can't find it.
