I don't have a problem per se with hands in front, but it always seems pointless if the girl's gagged if they can just reach up and take it off.
Also what's the point in going to the trouble of gagging your damsel in anime/cartoons, if all they need to do is a little wiggle and off it comes.

Thats why most gags in general are pointless and nothing more than superficial add-ons. without stuffing their mouth or locking there jaw in place with: a ball, ring, bit, or dental gag, it serves little purpose in preventing noise or talking
